Patients coming to the Duke Sports Medicine Center can now download patient history forms for their appropiate medical problem. Please click on the form that correlates with your injury/problem and complete and bring to clinic with you. (Please print single-sided only and fill out in ink). This will help expedite your clinic appointment with us.
